Community Health Center of Central Missouri was established in 2005 to serve the primary healthcare needs of Central Missouri residents. We realize that providing primary care services is much more than just providers and facilities. Quality of care, customer service, disease management, value and convenience are also key parts of the service delivery process. Community Health Center emphasizes providing access to primary care services to everyone, regardless of their ability to pay and works with our community partners to achieve our vision of “embracing people, inspiring health”. CHCCMO is a Federal Tort Claims Act (FTCA) deemed facility.

Today our vision continues to expand as we identify and put into practice beneficial health care program for our patients. pcmhWhat is a “patient-centered medical home”?

A patient-centered medical home is a model of care that strengthens the physician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has an ongoing relationship with a personal physician who leads a team at a single location that takes collective responsibility for patient care, providing for the patient’s health care needs and arranging for appropriate care with other qualified clinicians. The medical home is intended to result in more personalized, coordinated, effective and efficient care. A medical home achieves these goals through a high level of accessibility, providing excellent communication among patients, physicians and staff and taking full advantage of the latest information technology to prescribe, communicate, track test results, obtain clinical support information and monitor performance.

Community Health Center of Central Missouri currently holds a Level 3 PCMH Recognition. This is the highest level of achievement.
