Payment Options

Community Health Center of Central Missouri offers a Sliding Fee Discount based on the Federal Poverty Guidelines to assist uninsured patients in receiving quality care. The Sliding Fee Discount income guidelines are updated annually on the first of each fiscal year by the CFO with approval from the Board of Directors. These guidelines are updated each calendar year. Patients cannot be seen on the Sliding Fee program until they have completed the necessary paperwork and enrollment process. Once approved, the agreement will be valid for one year, at which time the patient will be required to re-enroll. At any time, while receiving assistance through the slide program, the patient is required to report any changes in household occupancy or financial status for re-evaluation.

Proof of income may include the following documentation:

  1. Two recent paystubs reflective of accurate hours and gross earnings (dated within 30 days of each other and not more than 90 days old)
  2.  W-2 Wage Statement
  3. Prior years Federal tax return (Form 1040)
  4. Statement of Benefits (Social Security, Food Stamps, TANIF)
  5. Employer earnings statement
  6. Federal or State Award Letter
  7. College grant award letter with total amount of award; student loans are excluded
  8. Child Support Statement

Except in urgent situations, please call several days ahead for an appointment. Preventive care, such as family planning, well-child exams and physicals, are usually scheduled in advance. If you need an appointment on the same day, please call early in the morning. We will do the best we can to see you.
